Sport Physical Information

A pre-participation physical examination is required for students to participate in Oxford Area High School athletics. To read the school's physicals policy, click here.

The physical exam must include completion of the following documents (Click on document names for Adobe PDF Files):

Sections 1 to 4 must be completed prior to any participation in OAHS athletics. Review carefully as parents must sign in multiple locations.

A family physician must conduct the examination and sign and stamp the back page. Physicals without a physician's stamp or verification phone number are invalid.

Provide as much accurate information as possible in the medical history (section 3). Parents will be notified in the event of an emergency.

This multi-use form gives consent for the Certified Athletic Trainer (ACT) to administer a single dose of over-the-counter medication when necessary. If the ATC does not have parental permission, he will not administer the medication.

Indicate which sport(s) your child plans to play. Fill out one column for a single sport, two columns for two sports, etc. Sign at the bottom of each column.

Note: If this is the fall physical, indicate your child's grade for the upcoming academic year.
